New Patient Registration

Anybody who lives within our catchment area may register with us. It's helpful if you can bring proof of identity (preferably your passport) and proof of address (for example, a bank statement or utilities bill) as this speeds up the process, but it's not necessary - anyone can register without ID.

Registration takes place Monday to Friday between 09:30 and 18:30 and 09:00 to 11:00 Saturday. Although you can register between these times the surgery is less busy after 18:00 and on Saturday making the process faster.

At registration you will be given a brief health check which involves answering some questions about your health and lifestyle and recording your blood pressure, height and weight.

You can download, print and complete the registration forms below, at your leisure, before you come in to register.

Please print both GMS1 and registration form before coming to the surgery to complete your registration.
