Register as New Patient
Anybody who lives within our catchment area may register with us. The best way to do this is to call and book a quick appointment with our admin team so that we can get everything sorted out in one go including a brief initial health check covering lifestyle and recording your blood pressure, height and weight.
It’s helpful if you can bring proof of identity (preferably your passport) and proof of address (for example, a bank statement or utilities bill) as this will speed up the process, but it’s not necessary – anyone can register without ID. For children (under 5 yrs) we also need to see the Birth Certificate and recommend that you bring the child’s Redbook.
You can download, print and complete a GMS1 Registration Form below, at your leisure, before you come in for your registration appointment. This will also save some time on the day.
